ENTRY FORM

1. What should be my minimum and maximum age to be eligible to participate in TEEN OF THE YEAR 2009?

You should not be below 15 years and above 18 years of age as on August 1, 2009.

2. How do I participate?

If you are within the above-mentioned age limit you may fill the prescribed application form along with the necessary supporting documents and a brief outline of the project you wish to undertake.

3. Does my entry form need to be attested by anybody?

For us to believe that you are an authentic student, you need to get your form attested by the principal of your school / college with the stamp of your school / college.

4. Do I need to attach any documents along with the entry form?

Photocopies of your certificates and other awards received only from class VIII onwards need to be attached to validate the claims made in your entry form. These too will have to be attested by the principal of your school / college along with the stamp of your school / college.
Thus, ensure that your form is filled carefully in detail, all your certificates / documents are attached, project outline is clearly comprehensible and everything is duly attested by the principal of your school / college with the stamp of your school / college.

5. Do I need to attach my photographs?

Yes. Your entry form should be accompanied by two recent 5 cm x 5 cm photographs - one stuck on the form in the place provided and the other neatly stapled to the form taking care that the staple does not come on the face.

6. If I have participated earlier, am I still allowed to participate?

No. Participation is allowed only once. Those who participated in any of the previous TEEN OF THE YEAR pageants (zonals or finals) are not eligible to take part.

7. What incentive do I get if I participate?

Whether you are selected for the zonals or not, as a participant, you will be entitled to receive your favourite magazine THE TEENAGER, absolutely FREE for one year from the October 2009 issue. Further, you will be entitled to participate FREE in other activities organised by THE TEENAGER.

8. Do I have to pay a registration fee and how and where do I pay it?

Yes, you will have to pay a Registration Fee of Rs 200/- by Demand Draft favouring THE TEENAGER, Mumbai along with your entry form.

9. What is the last date for sending in my entry?

The completed entry forms should reach THE TEENAGER, 58/23rd Road, TPS III, Bandra (West), Mumbai 400 050, latest by August 15, 2009.
Forms sent via e-mail will not be accepted or considered.
